What are the red flags of JIA?

Red flags in suspected JIA are features that warrant prompt assessment due to potential severity or alternative diagnoses. High, spiking fevers, widespread rash, marked fatigue, rapid weight loss, or night sweats may suggest systemic involvement. Severe joint swelling with inability to bear weight, significant limitation of movement, or intense pain should be evaluated urgently. Eye symptoms—redness, blurred vision, light sensitivity—raise concern for uveitis, though uveitis can also be silent, making routine screening essential in higher-risk subtypes. If symptoms persist, flares recur, or the child worsens alongside infection signs, a passive “wait and see” approach is not appropriate. Pediatric rheumatology evaluation is the strategically correct escalation step.
